Contract and Release of Academic Progress Information

 

For new ExSEL participants, this form is valid for the 2009-2010 and 2010-2011 academic years. For previous ExSEL Participants, this form is valid for the 2009-2010 academic year. See the ExSEL Brochure or website, www.exsel.mtu.edu, for descriptive information.

As an ExSEL Program participant, I agree to:

 

Attend all of my classes on a regular basis Enroll in the UN1000 “Frameworks for Success” course

 

Enroll in the UN1000 “Frameworks for Success” course

 

Put forth my best effort in completing homework assignments

 

Meet with a Peer Mentor Group once a week, for 5 weeks

 

Attend tutoring as needed throughout the year

 

Meet with the ExSEL Coordinator or staff as required (maximum of once per week), including a one-on-one meeting with ExSEL Coordinator or staff during spring semester.

 

Grant access to my parent(s)/guardian(s) to view my grades online using the Banweb Guest Access System at Michigan Tech. This will allow the ExSEL Coordinator and/or staff to discuss my progress with my parent(s) or guardian(s).*

 

Give permission for the ExSEL staff to send out notices to the people who I have granted access when my grades are available and discuss my academic performance with the person(s) I have granted access to.

 

Participate in surveys and other assessment tools that will support the ongoing improvement of the program

For our part, at Michigan Tech, we agree to:

 

Assist you in making the transition to college life and to Michigan Tech Be available to your parent(s)/guardian(s) to discuss issues and concerns they may have regarding your progress or transition

 

Be available to your parent(s)/guardian(s) to discuss issues and concerns they may have regarding your progress or transition

 

Provide assistance through the ExSEL Coordinator, staff and academic peer mentors who understand the issues that first year students face

 

Identify appropriate campus resources (such as tutoring sessions) to help you succeed

* If participants choose to not provide Banweb guest access to their parent(s), guardian(s) or other individuals, ExSEL, program staff is, by law, not permitted to discuss a participant’s academic performance with those outside parties.
